Instructions

  1. All information on this form is REQUIRED.
  2. Unless otherwise specified, all information is about the player.
  3. You can submit this form electronically. However, if you prefer, you can complete it, print a copy, and mail it to:

  4. Fall-Winter fee is $900 for Middle School and $800 for High School. Pay $50 less if all fees paid by 9-9-2009. Payment schedule: $200 with registration; $300 by 10/31; $400 (MS) or $300 (HS) by 12/15
  5. Iceline League requires we put "pure" teams on the ice for teams, however, some exceptions have been made in the past. So if a player does not attend a public school in the Owen J Roberts School District please send a note to us with your eligibility situation or any questions.
  6. By league rule, membership with USA Hockey is mandatory.
    • This includes players and coaches.
    • Coaches will verify the registration number before any player is allowed to go on the ice the first time.
    • The league will also verify USA Hockey registration.
  7. Uniforms are not automatically ordered based on the registration.
    • High School uniforms (a home and an away jersey, each with last name on back, and a home and an away pair of socks) must be purchased separately through the team. Cost is approx. $80 for both jerseys and $22 for both sock pairs.
    • Team jersey numbers must not conflict, so do NOT order a uniform until all registration information is processed by the team and you are notified.
    • Every effort is made to accomodate preferred uniform numbers. The team uses the following guidelines:
      • Returning players have rights to their numbers - or to change numbers.
      • First come, first serve for balance of available numbers.
  8. Waiver forms will also need to be signed by a parent or legal guardian for each player, each season in order to complete the registration process. They will be available for download on the team site or at the first game or practice.
  9. When done with form, click the "Verify" button at the bottom of the page. This will not process the registration, but will verify all information is provided and prevent spam. The club will then follow up with you via e-mail or phone.
